Care Navigation
Care Navigation (palliative care) addresses the physical, psychological, social and spiritual needs of patients and families with serious chronic and/or malignant illness. The focus is on improving the quality of life for our patients. It is a comprehensive consultation with patients and families working together to identify the specific needs of the patient and finding the resources available to address those needs adequately. There are no fees for the consultation.
Lakeview Hospital's Care Navigation team is available to assess the symptom management and comfort needs of patients. A consult can include the patient, members of the patient's family, the patient's physician and one or more members of the Care Navigation team. Services include pain and symptom management, and psychosocial support and counseling. We also provide education in homecare and hospice services and offer referrals to community resources.
