If you or someone close to you has a serious illness with symptoms that are not well controlled, or if treatment is no longer working, our Palliative Care Navigation program may be right for you. Palliative care describes a health care program which seeks to improve the quality of life for patients and families who are facing serious or advanced illness. Palliative care is close attention to the emotional, physical, spiritual and social aspects of illness.
Our Palliative Care Navigation team includes:

- licensed practical nurses
- home health aides
- social workers
- physical therapists
- occupational therapists
- speech therapists
- chaplains
- pharmacists
- registered dietitians
- volunteers
- bereavement coordinator
The Palliative Care Navigation team offers:
- management of difficult symptoms (pain, nausea, anxiety, fatigue, depression, shortness of breath, constipation, loss of appetite, other symptoms)
- addressing changing goals of care (helping you consider care options, interpreting the benefits/burdens of care, helping determine when/if to begin/discontinue treatments)
- enhancing patient and family support (identifying what is important to you, supporting communication, providing spiritual and emotional support, promoting well-being and peace of mind, promoting the best quality of life, helping all involved create meaning)
